Open Tibia servers — OTS — are independently run and report their own numbers, and they are where most of the customisation lives: custom rates, custom spells, war servers, and revivals of long-dead world types. PvP type is the filter that changes the game most: optional PvP, open PvP, retro hardcore and everything between are all represented, and joining the wrong one is the fastest way to lose a character.

Continuously. A vote changes the ranking the moment it is counted, and player counts are refreshed every few minutes. Monthly vote totals reset at 00:05 UTC on the 1st, so every list starts each month from zero and lifetime votes keep the order sensible until new votes come in.
